2023 Surplus Rebate Forms will be available starting November 1st, 2023. You may only obtain an application at the Crestwood Village Hall located at 13800 S. Cicero Ave.

In order qualify for the 2023 Surplus Rebate, the following requirement must be met;

1. You have moved in and resided in your home on or before 1/1/2022.
2. You are currently residing in the home.
3. You have paid your 2022 Property Taxes.
4. You are the owner of the home and your name is listed on your property tax bill.
5. You receive no rental income from your property.
